OSHA Form 301

What is OSHA Form 301?

OSHA Form 301, titled “Injury and Illness Incident Report,” is required by the Occupational Safety and Health Administration whenever a recordable workplace accident, illness, or death occurs. Employers must complete this form with detailed information about the incident, including how it happened and what treatment was provided.

Why OSHA Form 301 Matters

Form 301 works alongside OSHA Form 300, the log used to record and categorize all such incidents at a workplace. Every event noted on OSHA Form 300 must have a corresponding, more thorough report on Form 301. Together, these records help employers track safety trends, comply with federal regulations, and identify areas where workplace conditions need improvement.
